• To receive a price quote and proceed with testing, the customer will receive specific forms with all necessary details, which require physical signature (hereinafter: “the Forms”).

4. Payment on the Website

• Upon receipt of the signed forms, payment for the test can be made either by phone through the Company’s customer service center or by filling out a payment form on the Website.
• Only individuals over the age of 18 or legal entities incorporated under applicable law may complete forms and make payments on the Website.
• If the transaction is not approved by the credit card company, the customer will be notified via the payment link and required to resolve the issue. Without such resolution, the transaction will not be completed, and the Company will not be bound by the previously offered price.

5. Transaction Cancellation Terms

• Transaction cancellations will be handled in accordance with the Israeli Consumer Protection Law and the terms outlined in the signed forms.

6. Privacy and Information
